Wow, gorgeous AND smart!?! I'm guessing you're here because you saw my BLOG POST about the perks of being a VIP Senior Model and you can't wait to be a part of it!

The purpose of this form is to get to know you a little better and see if this program would be a good fit for you and your family. There are no "right" or "wrong" answers so please answer honestly and completely. Due to our wonderful response rate, incomplete applications will not be processed.

Please take a few extra minutes to respond to the short answer questions and share any additional information you would like for us to consider. We love to create custom sessions and get inspiration from you!

If accepted into the program, I agree that I will not represent or be photographed by any other studio or photographer (excluding those that are required by my sports or school activities).

If accepted into the program, I agree to allow The Proof Photography to use and publish the photos and videos taken during my session(s) for print and online marketing.

I have permission from my parents or guardians to apply to be on the The Proof Photography Model Team. I have shared all the information with them and they are aware there is a $395 session fee which covers multiple sessions if I am accepted to the Senior Model Program.
